Description

The Intake Clinician is responsible for all aspects of the initial assessment of members and their family situation for intake into appropriate services, including gathering data for a comprehensive psychosocial assessment, making clinical diagnosis, and completing the service plan. Intakes performed may be for both adults and children. The individual in this position must meet all established guidelines and requirements including the timelines for referrals and written documents. The individual also must successfully complete all required training identified by the RBHA and EBF and meet the credentialing and privileging requirements as descripted by AHCCCS.

Essential Duties and Responsibilities:

  • In an office, home and/or community based setting as appropriate the Intake Clinician:
  • Receive DCS referrals and make contact with childrens placement within 24 hours.
  • Complete the Rapid Response assessment within 72-hours of referral.
  • Conducts a comprehensive psychosocial assessment.
  • Makes treatment recommendations.
  • Summarizes primary presenting issues.
  • Formulates a diagnosis.
  • Provides effective crisis assessment.
  • Completes all necessary paperwork related to enrolling an individual into the Easter Seals Blake Intake and Coordination of Care Program.
  • Refers members to appropriate community resources as necessary.
  • Provides Individual and Family Therapy as requested and appropriate.
  • Attend and participate in team decision making meetings.
  • Attend and participate in Preliminary Protective Hearings.
  • Complete progress notes in HMS within 24 hours of service.
  • Communicate with DCS, placement, biological parents and any other team member to initiate/coordinate services.
  • Respond to crisis calls and performs other additional services, as needed.
  • Engages families and offers services, as appropriate.
  • Attends and participates in Child and Family Team and Adult Recovery Team meetings as needed/required.
  • May carry a case load of therapy based on agency need.
  • Attends and participates in required staff meetings.
  • Submits accurate and complete written reports within required timelines as identified by the RBHA and AHCCCS.
  • Develops and maintains positive relationships with program participants, staff, other programs and state agencies.
  • Maintains agreed upon work schedule, is punctual, keeps appointments, and uses time effectively.
  • Uses supervision effectively, accepts feedback in a professional manner, and accepts responsibility for job performance.
  • Communicates essential information with ESBF team staff and team members accurately and in a timely manner.
  • Understands and maintains confidential information within the confines of HIPAA.
  • Performs any other duties as assigned and appropriate for the position.
  • Responsible for meeting productivity and documentation requirements set by the supervisor.
