Sevita Physical Therapist Assistant in Worcester, Massachusetts

MENTOR Adult Day Health, a member of the Sevita family, provides day programs for elders with complex medical or rehabilitative needs. This allows seniors to obtain quality healthcare while remaining active and involved in the community and continuing to live in their own homes. Our programs are staffed with qualified, experienced licensed nurses and caregivers who are trained to provide individual assistance to each person we serve.

ACE Medical Services, a member of the Sevita family, is hiring a Physical Therapist Assistant for our Home Care department.

A home care physical therapist assistant works under the supervision of a physical therapists who helps treating patients that are suffering from physical disability in a home care environment. These physical disabilities are often due to injury, disease or surgery. Their typical duties and responsibilities include:

  • Provides direct physical therapy treatment to patients according to instruction of Physical Therapist, and in accordance with the written plan of care.

  • Observes patient and reports to Physical Therapist and department personnel the patient’s reaction to treatment and any changes in patient’s condition.

  • Instructs patients, family and/or caregiver in proper techniques to assist patient in achieving maximum level of independence.

  • Maintains necessary records, progress notes, and conference notes, which are incorporated into the patient’s clinical record within established time frame

  • Maintains professional standards, compliance with State, Federal (CMS conditions of participation) and Joint Commission regulations, and infection control protocol by participating in in-services, staff meetings, process improvement teams, continuing education seminars when available, and adhering to the expectations of Sevita Health – standards of care guidelines.

Qualifications:

  • High School Diploma or equivalent.

  • One year of experience in a human services or related field.

  • Current PTA license in the state of work.

  • Good working knowledge of physical therapy equipment and therapeutic exercise treatments.

  • Self-motivated and detail-oriented with ability to multi-task.

  • Reliable, responsible, and caring nature.
