Promotes individual and population health by leading community health planning initiatives. This includes facilitating and developing a community health assessment in collaboration with other department staff, hospital systems, community stakeholders, elected officials, community members and other agencies and organizations. Also includes implementation and evaluation of the community health improvement plan. This position seeks out revenue sources and conducts frequent and ongoing grant applications to support efforts to address public health priorities. Essential Duties and Responsibilities Public Health Sciences 1. Develops and implements timelines and master planning for community health assessment and planning activities in order to assure the department\'s certification as a level III health department in Wisconsin and to comply with National Voluntary Public Health Accreditation Standards. 2. Facilitates and implements assessment and planning, including selecting the framework for this process. 3. Understands and articulates the Foundational Public Health Services. 4. Applies public health sciences to programs and policies. 5. When determining goals, objectives, and strategies, conducts a comprehensive review of the scientific evidence related to the public issue, concern, or intervention. Community Dimensions of Practice 6. Develops and implements a process that assures the active involvement of community members and stakeholders in the community health assessment and planning process. 7. Leads the design and application of community health improvement strategies with community partners, members, and stakeholders. 8. Collaborates with academic partners on sanctioned research and demonstration projects that seek to improve the health of communities and determine new ways to address health issues. 9. Negotiates for the use of community assets and resources. 10. Facilitates development and provides leadership of community workgroups and committees related to community health assessment, prioritization, planning, implementation, and evaluation. 11.
