SUPERVISES No supervisory responsibility. RESPONSIBILITIES The following duties are normal for this position. These are not to be construed as exclusive or all-inclusive. Other duties may be required and assigned. ESSENTIAL FUNCTIONS 1. Coordinate, schedule and conduct intake, risk/need assessment, evaluation and orientation with new client admissions. 2. Utilizing a comprehensive risk and need evaluation develop, implement and revise a case management plan in accordance with agency policy. 3. Provides interventions for family members and significant supports of persons receiving/needing treatment. 4. Identifies, develops, adapts and implements case management methods according to client needs. 5. Maintain client files in accordance with applicable agency, County, State and Federal guidelines. 6. Coordinate appropriate referrals. 7. Advocates on behalf of clients to assure client needs are met appropriately and in a timely fashion, through meeting with service providers/administration to explain client needs and to facilitate service delivery. 8. Meets with other community agencies/organizations, as directed, to assist them in providing relevant programs/services for clients or to develop programs to meet other community needs. 9. Gathers and provides to administration such information as may be deemed useful for ongoing program evaluation and planning. 10. Attend regularly schedule staffing\'s and court appearances of program participants. 11. Researches and implements, as directed, treatment court programming like mentoring and alumni groups. 12. Promptly processes referrals to treatment court by completing treatment screens efficiently and within time frame established by the agency. 13. Perform regular, random and observed drug screens in accordance with agency policy 14. Assures incentives and sanctions are given on a consistent and fair basis 15. Recommend evidence based therapeutic incentives and sanctions 16. Assists in seeking potential funding sources 17. With other team members, attends local civic organizations, bar association and other community meetings. 18. Ensures data is collected and documented within the time frame established by the agency. 19. Responsible to know and practice the Safety policies of the County. Perform all job tasks in a safe and prescribed manner. 20. Any other duties as may be assigned. ***** OTHER EXPERIENCE AND QUALIFICATIONS: -Minimum of two (2) years working within the addiction and/or community corrections field or, any combination of experience and education that meets the r quirements of the position. -Comprehensive knowledge of the criminal justice system as well as substance use disorders and co-occurring mental health disorders is preferred. -Possession of a valid driver\'s license, as well as a licensed and insured automobile are required as a condition of employment. Traveling and working in both Wisconsin Rapids, WI and Marshfield, WI is required. -Must be cleared to access CJIS (Criminal Justice Information Services) reports and DOC (Department of Corrections) records per FBI (Federal Bureau of Investigations) and DOC eligibility requirements. -Must be allowed clearance to access the Wood County Jail per the Sheriff\'s Department. -Basic everyday living skills, including the ability to understand and follow directions (such as court procedures and protocol, statutory requirements, etc.), and reading and writing is necessary. -Ability to communicate orally and in writing with individuals and groups. -Ability to maintain confidentiality. -This position will require weekend hours; however the schedule is flexible.
