Job Purpose:The Phlebotomist is responsible for assisting HealthCore Clinic Providers to provide high quality and efficient patient care and skillfully performs venipuncture and skin punctures on patients. The Phlebotomist is also responsible for performing such services and utilizing knowledge of age related factors, specimen requirements, specimen volume, aseptic technique and universal precautions.

Patient-Centered Medical Home Essential Functions:

1.Patient Population Management:

a.Labels and records all test tubes, log procedures and file paperwork.

b.Documents immunizations for all vaccines given to patients.

2.Care Coordination:

a.Obtains blood of sufficient quantity and quality from adult and pediatric patients utilizing proper phlebotomy and identification techniques including venipuncture, heel, toe or finger sticks.

b.Verifies samples are taken from the correct patients by identifying patients before collecting specimens.

c.Processes specimens brought to the laboratory and delivers the specimens to the correct section for testing.

d.Utilizes the laboratory computer system to process and track specimens to ensure efficient pre-analytic specimen processing.

e.Obtains cultures for Microbiology and Virology using aseptic techniques and anatomy knowledge to select the proper site and obtain accurate results.

f.Performs bleeding time tests at the patients bedside when provider request is received.

g.Fills orders and tracks location and status on specimens; performs appropriate follow up with patients.

3.Self-Management Support:

a.Provides self-management support to patients by providing self-management tools and encouraging patients to self-manage and promote healthy behavior changes.

b.Complies with all applicable federal, state, local and clinic rules, regulations, protocols and procedures.

c.Utilizes universal precautions and knowledge of more restrictive isolation procedures when procuring, transporting and processing specimens to ensure patient and employee safety.

4.Communication Skills:

a.Communicates effectively with all patients either in person or over the phone and displays excellent customer service to all patients, clients and visitors.

5.Quality Improvement:

a.Participates in the improvement of operational functions of the department.

b.Assures personal compliance with infection control, personal protective equipment, blood borne pathogens and hazardous material handling.

Additional Essential Functionsinclude the following.

1.Stocks lab area with supplies as needed.

2.Cleans and sterilizes equipment.

3.Assists Providers by completing various duties as instructed.

Additional Responsibilities:

1.Other duties as assigned.

Qualifications and Requirements:To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Phleb tomy certification required.

Must possess a current CPR certification and keep the certification up to date. If the employee does not have a CPR certification, one must be obtained within thirty (30) days of employment from start date.

One (1) to three (3) years previous experience as a phlebotomist highly preferred.

Have a positive, friendly and professional demeanor with excellent interpersonal skills.

Assist with gathering data and inputting data into databases.

Strong computer literacy, especially with medical center software.

Strong office skills including accurate typing, data entry, and filing.

Attention to detail and ability to compose and generate documents and reports.

Ability to communicate effectively with vulnerable populations including those made vulnerable by their financial status, personality characteristics, place of residence, health status and/or age.

Ability to multi-task and organize work effectively while working independently and/or as a cooperative team member.
