Position : Client Care Manager

Location : Our office is in Wheat Ridge. Client visits are throughout the West Denver Metro area - Lakewood, Golden, and Arvada.

Key Responsibilities :

Client-Centric Focus : Build and maintain warm, empathetic communication channels with clients, their families, and caregivers.

Assessment & Planning : Conduct thorough in-home consultations and manage the process to bring new clients on board. You will also create personalized care plans that champion our client’s needs and well-being.

Collaborative Teamwork : Work seamlessly with our team, ensuring the right caregiver-client matches and facilitating smooth introductions.

Continuous Improvement : Stay proactive in providing ongoing education and resource recommendations to clients and their families, ensuring they feel supported at every stage.

Networking: Build and maintain relationships with select local referral sources (e.g. hospitals, assisted living).

Who We Are Looking For:

Adaptable Team Player : Someone open-minded, adaptable, and able to thrive in a close-knit team environment.

Outstanding Communicator : Proficiency in building and maintaining strong, supportive relationships.

Problem Solver : Ability to navigate challenges independently, always seeking the best solutions for our clients.

Technical Proficiency : Comfortable using modern software applications (Microsoft apps, Google apps, Slack/Teams, etc).

Educational Background : A Bachelor’s degree is preferred. Experience in home care, social work, or a related field is a plus, but not mandatory.
