Description

MAIN FUNCTION:

  • To oversee the coordination and provision of timely discharge/continuing care planning for patients in treatment and patients going to Seafield sober homes, thereby enhancing the next level of care.

  • This position is classified as Essential Day 2 for emergency closure purposes.

    ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Schedules and conducts individual discharge planning session with patients.

  • Enter progress notes and other clinical documentation, including discharge bundle, into the EMR.

  • Coordinate patient transportation upon discharge.

  • Interfaces directly with case managers as to patient’s input/commitment to this process.

  • Act as liaison between this facility and facility projected to provide next level of care.

  • Schedule outpatient appointment on EMR scheduling calendar.

  • Make referrals to appropriate community services and outside providers.

  • Explores usage of potential continuing care service providers.

  • Coordinate a telehealth appointment for current patients and Seafield outpatient facilities

  • Conduct screenings and approvals for patients going to Seafield recovery housing

  • Handle the placement of all patients going into Seafield managed sober homes

  • Handle the admissions and discharges of all patients in Seafield managed sober homes

  • Forward weekly census to house managers and outpatient facilities.

  • Maintain accurate, up to date, house census and keep outstanding Resources balances.

  • Screen and answer calls from potential and past or present house residents and referral sources.

  • Liaison with Outpatient facilities regarding potential admission placement.

  • Screen outside provider biopsychosocial evaluations for housing admission criteria.

  • Maintain accurate, up to date, house census and keep outstanding Resources balances.

  • Forward weekly census to house managers and outpatient facilities.

  • Office duties to include – typing memos, letters, correspondence and reports as needed.

  • Maintain daily contact with house managers.

  • Complete monthly housing referral source report.

  • Perform other duties as assigned.

    COMPETENCY REQUIREMENTS:

  • Knowledge of referral sources

  • Individual counseling

  • Discharge and Referral

  • EMR documentation

  • Proficient in computer usage including EMR System, Microsoft Outlook and other information systems.

  • Followup calls

    EDUCATION AND QUALIFICATIONS:

    Bachelor’s Degree preferred and minimum of one-year experience in discharge planning. CASAC preferred. Supervisory experience preferred.

  • While performing this job, the employee is required to talk and hear. Specific vision abilities required by this position include close vision, distance vision and the ability to adjust focus.

  • This is a largely sedentary role and involves sitting most of the time. Walking and standing are required occasionally.

  • The employee must have the ability to use a computer keyboard, mouse, calculator, copier, fax machine, scanner and telephone.

    Full Time Availability $19-$21
