Mon - Fri : 8 am to 5 pm Wage: $23.32 - $28.62 Benefits * Medical/Dental * FSA * 403(b) Retirement * Additional Voluntary Benefits * Paid Holidays * Vacation/Sick Leave * Education Assistance Program * Employee Assistance Program * Longevity awards Position Summary: The Assisted Outpatient Treatment (AOT) program is designed to reduce the frequency and duration of psychiatric hospitalization, emergency room admission, homelessness, incarceration, and interactions with the criminal justice system while improving the health and social outcomes of individuals with serious mental illness. Under the general supervision of the Associate Director of Crisis Services, the Assisted Outpatient Treatment (AOT) Case Manager provides professional services in accordance with person-centered planning and coordination of support services for adults with serious and persistent behavioral health disorders who need assistance in accessing, coordinating, and monitoring resources and services. The Case Manager supports and assists individuals in developing independent living skills and rehabilitation goals, provides liaison to other mental health agencies and professionals, and works with family members and others in the client's support system. Case Management services also include crisis intervention, relapse prevention support and emergency services, ongoing psychosocial assessment, family support services, and vocational support. Responsibilities: Provide on-going monitoring of individuals to ensure compliance with each AOT court order Provide linkage and service coordination to individuals to appropriate/court order treatment modalities Assist with obtaining public entitlement and benefits such as Social security, Medicaid, Medicare, welfare, food-stamps and Section 8 Assist with the integration of mental and physical healthcare and developing a Psychiatric Advance Directive. Supervise, organize and support recreational and/or socialization activities Provide transportation, support counseling, crisis intervention services as needed Establish and maintain an ongoing therapeutic relationship and case load as skill level permits Monitors service delivery to meet program participants changing needs Provides case management to individuals Must be able to communicate effectively and participate in both verbal and physical de-escalation techniques. Ability to excel in a stressful, constantly changing environment is required. Completes progress notes and other required documentation in the electronic consumer files within the required timeframe. Monitor service delivery and compliance with court ordered treatment Inform supervisor, and other members of the team as indicated, of issues/concerns related to individual clients Adhere to all Agency Policies & Procedures. Provides training and guidance to new staff in addition to current staff that need more direct assistance.
