Downtown Streets Team Flamingo Motel Case Manager in West Sacramento, California

COME CREATE IMPACT WITH US Downtown Streets Team (DST) is looking for a passionate and outgoing Case Manager at the Flamingo Motel Emergency Shelter site who is ready to help us end homelessness through the dignity of work and the power of community. SUMMARY: We’re looking for a full-time Case Manager to work directly with individuals experiencing homelessness, who are currently residing at the City of West Sacramento’s Flamingo motel conversion emergency housing project, to help them navigate social services and reduce barriers to self-sufficiency. This may include helping residents access employment, housing, medical, legal, financial services, and other goals. You will be facilitating life skill and employment skill workshops regularly to both individuals and groups. You’ll need strong organizational, problem-solving, analytical, and people skills while managing priorities and workflow. WHAT YOU'LL DO: Help Motel Guests Achieve Self-Sufficiency Provide compassionate support and assistance to each motel guest to achieve their goals on housing, jobs, transportation, health, and legal issues Work with motel guests to develop job search collateral, run employment readiness workshops, and employment-focused volunteer pairing events Develop housing and employment leads with local landlords, property managers and employers Utilize any available subsidies and affordable housing units to place motel guests into housing Maintain regular appointments and office hours with motel guests and staff Present new services and opportunities to the residents during designated Case Management sections in a monthly resident meeting Support motel guests in court, at housing related appointments, social service agencies, etc. This may include providing them with transportation Establish productive relationships with each and every motel resident with no bias or prejudice towards race, religion, age, sex, disability, or sexual orientation Work Well with Others in DST & the Community Work with the Project Manager to promote successful participation in the emergency shelter program Actively document client meetings and update all relevant program data in applicable databases such as DST Tracker and Yolo County HMIS Uphold organizational values, work ethically and with integrity, and follow company policies as laid out in the employee handbook Represent DST in the community, and work with all partner organizations to achieve client goals Distribute outreach supplies to individuals in encampments, RVs, and on the streets, and promote participation in various support services available in West Sacramento Actively learn about current resources and services outside of DST that may aid motel residents in their goals Perform other duties as assigned by manager and assist with special projects as required WHAT YOU'LL NEED: Ability to perform job duties with minimal supervision Excellent communication skills, with fluency in written and spoken English Basic mathematical calculations Problem solving and observation skills Self-starter with strong organizational skills with the proven ability to multi-task, prioritize duties, and manage time Works well with others and behaves professionally and ethically Solution-oriented Ability to work in a changing environment High school diploma/GED Valid CA Driver License, reliable vehicle with current insurance, and the ability to travel regularly throughout the local area Physical requirements: sitting, standing, or walking for long periods of time and pushing, pulling and/or lifting up to 25 lbs WHAT WE CAN OFFER YOU A chance to positively impact your community on one of the biggest domestic challenges of the day Competitive nonprofit compensation Options for fully paid medical, dental, vision and life insurance coverage for employees, with FSA and HSA health plans available Option to enroll in 401k after 90 days, and up to 6% company match, after one year of employment 15 days off PTO, increasing with tenure Over 10 paid holidays annually Paid company closure for five days during the summer Paid company closure for five days between Christmas and New Year Day off on your birthday One month paid sabbatical after five years, not charging PTO Internal growth opportunities Access to professional development resources and reimbursement Employee Assistance Program (EAP) WORK ENVIRONMENT: Moderate noise associated with an open office work environment.
