Summary:

  • The Oncology Infusion Pharmacist is responsible and accountable for the provision of safe, effective and prompt medication therapy in the ambulatory infusion centers. The Oncology Infusion Pharmacist collaborates with providers and nurses to ensure safe and effective chemotherapy and adjunctive medications for ambulatory oncology and non-oncology infusion patients. The Oncology Infusion Pharmacist is responsible for overseeing both hazardous and non-hazardous sterile compounding and ensuring compliance with regulatory requirements. The Oncology Infusion Pharmacist collaborates with physicians, advanced practice providers, nurses, and other members of the healthcare team to deliver optimal medication therapy to patients with a broad range of disease states. Pharmaceutical care services include, but are not limited to, assessing patient needs, incorporating age and disease specific characteristics into drug therapy and patient education, adjusting care according to patient response, and providing clinical interventions to detect, mitigate, and prevent medication adverse events. The pharmacist provides medication and practice-related education/training by serving as a preceptor for doctor of pharmacy students and PGY1 and PGY2 pharmacy residents.

Responsibilities:

  • Responsible for ensuring safety in the compounding and dispensing of chemotherapy, maintaining an adequate supply of medications, minimizing drug waste, minimizing unnecessary exposure to hazardous drugs, and managing cost and reimbursement for cancer drugs.

  • Collaborates with providers to deliver a wide variety of services to patients, including education, monitoring for adverse effects, evaluating drug—drug and drug–disease interactions, and providing supportive care.

  • Provides education to patients and caregivers on treatment regimens and side effect management.

  • Participates in local and health-system committees evaluating formulary management, treatment protocols, compliance with regulations and improvement of patient outcomes.

  • Collaborates with research staff to provide investigational drug services.

  • Serves as an educational resource for pharmacy students, residents and other health professionals.

  • Participates in pharmacy operations, medication dispensing, and oversees pharmacy technician activities.

  • Completes other duties as assigned to support the department, entity, and health-system.

Education or Equivalent Experience:

Required:

  • Doctor of Pharmacy Degree

  • 1+ years of Oncology experience

  • Registered Pharmacist in PA

Preferred:

  • PGY2 Oncology Residency

  • Board Certification in Oncology Pharmacy
