Description

Position Type/Expected Hours Of Work: 40 hours per week; flexible to the needs of the program. Hours may involve weekends and evenings depending on the needs of the department.

Qualifications: Bachelors’ Degree in Human Services or related field and two (2) years’ experience working in the field of developmentally disabled in a supervisory capacity OR Associates’ Degree in Human Services or related field and five (5) years’ experience working in the field of developmentally disabled in a supervisory capacity. Valid New York State Driver’s License required with acceptable driving history.

Work Authorization/Security Clearance: Authorized to work in the United States and pass all background clearances.

Summary/Objective: Assists in ensuring that all residential and day habilitative sites meet agency policy and procedure, OPWDD regulations and any other certification requirements that the agency adheres to such as CQL. Assess and ensure that the services received by people we support are provided in as natural an environment as possible. Develop an awareness of people we support as active citizens of the community.

Essential Functions: Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.

  • Assist in ensuring that all assigned facilities meet regulatory and other certification requirements and participate in site visits and agency reviews.

  • Complete and review quality services reports, determining if issues raised require further review and follow up with QS, Residential Managers (RM’s), Assistant Habilitative Supervisors (AHS’s), Interim Assistant Director and/or Director of Residential Services.

  • Visit all assigned sites (both residential and day habilitative) on a monthly basis, attend monthly staff meetings.

  • Meet with the Interim Assistant Director of Residential and Habilitative Services as scheduled to review monthly coordinator checklist and to review monthly financials for each assigned site.

  • Meet with RM’s and AHS’s on a regular basis, address issues expediently, establish objectives and implement corrective action if required.

  • Assist in the review /approval of all purchases within the assigned sites.

  • Review all work orders prior to submission.

  • Review Therap information and documentation on a daily basis as it pertains to assigned sites.

  • Ensure staff at sites are competent in the performance of their duties, provide staff with direction and assistance when required.

  • Review staff schedules for appropriateness in regards to preferred and maximum hours required for site level of supervision; maintaining awareness of sub utilization and overtime.

  • Coordinate the investigation of reportable incidents and complete written preliminaries required for submission to the Director of Residential Services.

  • Be familiar with the waiver regulations; 633, 624, 671, 483, 635.

  • Ensure that a daily safety inspection of your sites is conducted to minimize and address any possible safety hazards, and to ensure all equipment is being utilized safely and properly.

  • Other duties as assigned by the Director of Residential Services or Interim Assistant Director of Residential and Habilitative Services (JC).

    Supervisory Responsibility: Directly supervises Residential Managers and Assistant Habilitative Supervisors in assigned areas. Indirectly responsible for all Residential and Habilitative Services staff.

    Travel: Travel is expected for this position to locations in county in which you work on a regular basis and occasionally to other areas for work purposes such as training, meetings, or events.

    Work Environment: This job operates out of the Residential Services Office and out of the home of the people we support. This role routinely uses standard office equipment such as computers, phones, photocopiers, fax machine, filing cabinets and other regular office supplies. Visits are required to all program sites where services are provided under supervision of coordinator.

    Physical Demands: The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee is regularly required to talk or hear. The employee frequently is required to stand; walk; lift; use hands to finger, handle or feel; and reach with hands and arms. Work may require lifting, pushing, or pulling in excess of 30 pounds.
