Trinity Health Registered Nurse Urology Nurse Navigator (40 Hours) in Waterbury, Connecticut

Description:

Position Purpose : the Nurse Navigator is responsible for the navigation, development, and coordination of surgical patients referred to the Urology Service Line.

What you will do:

  • Coordinating care for patients to streamline comprehensive surgical care for each patient

  • Communicates effectively (written and verbal) with colleagues, patients and family members

  • Educate patients and family members throughout the process of diagnosis and treatment, providing pre-procedure teaching and post-procedure follow up calls.

  • Understanding of computer-based applications (E-MAR, clinical documentation), email, and standard applications such as Microsoft Office.

  • Collaborates with required services, including radiology, pathology, laboratory, and primary care & referring offices to provide seamless care for patients

Minimum Qualifications:

  • Graduate of an accredited School of Nursing. Bachelors of Science in Nursing (BSN) degree

  • Current licensure as a registered nurse (RN) in Connecticut

  • 3 years of practical experience in surgical nursing and familiarity with new trends and practices in surgery

  • BLS certification required

  • National certification in area of specialtypreferred

  • Standard development and management capabilities

  • Experience in a multispecialty environment preferred.
