Job Title: Care Manager (RN)

Job Description

Develops, assesses, and facilitates complex care management activities for members with primarily physical needs to ensure high quality, cost-effective healthcare outcomes. This includes creating personalized care plans and providing education for members and their families. The role involves evaluating member needs, overcoming barriers to care, and focusing on priorities identified by the member to recommend the best outcome plan. The Care Manager will develop ongoing care plans, collaborate with providers to identify necessary specialists and/or community resources, and coordinate between members and care teams for optimal, person-centered care. Responsibilities also include ongoing follow-up, monitoring changes in member conditions, providing support for local resource access, ensuring timely care delivery, and maintaining compliance with relevant regulations. Additionally, the role requires educating members and caregivers on disease processes, care options, and healthcare benefits, as well as providing feedback to leadership on care and quality improvements.

Hard Skills

  • Registered Nurse with CA or compact license

  • Care management

  • Medicare and/or Medicaid experience

Soft Skills

  • Ability to develop personalized care plans

  • Strong coordination and collaboration capabilities

  • Effective communication and educational skills

Work Site

This is a fully remote position.

Work Environment

Remote position with work hours from Monday to Friday, 8am-5pm, with the flexibility to start at 7am if needed. The role includes managing 5-10 referrals daily with a case load requirement of 75 active cases, ramping up over time. Each case typically spans 90 days.
