Blue Mountain Heart to Heart Case Manager, Recovery Navigator Program (Walla Walla) in Walla Walla, Washington

Case Manager, Recovery Navigator Program (Walla Walla)

This position coordinates with other case managers, an outreach manager, and supervisors on the Law Enforcement-Assisted Diversion (LEAD) Model Case Management team, supporting people with behavioral health issues in Walla Walla and Columbia Counties. The case manager will also closely liaise with mental health providers, opioid treatment staff, and outside partners in behavioral health. Starting salary range for this position is $53,500--$57,500 gross compensation per year, plus BMHTH benefits listed above.

Duties:

  • Understand and adhere to national LEAD Model guidance for case management and trauma-informed care practices
  • Ensure confidentiality of all Blue Mountain Heart to Heart clients, current and former
  • Provide intensive case management and referral services for health care needs, social safety net services, and other appropriate care as provided by BMHTH and partnering agencies
  • Provide timely, accurate, and complete reporting on cases in Julota or other health records systems, subject to regular review by supervisory staff, funding organization staff, and/or Executive Director or their designee.
  • Offer wraparound services to clients for housing support, food and nutritional assistance, workforce re-entry assistance, mental health support, substance use treatment referrals, and other support as needed
  • Liaise regularly with program stakeholders on the Operations Work Group, and communicate with them regarding client progress
  • Attend LEAD case management training as requested and required by funding organizations

Expectations:

  • Timely reporting to work
  • Clear written and oral communication with supervisor, coworkers, clients, and contract holders
  • Regular visits with clients, either in the office or at their homes
  • On-time, accurate, and complete paperwork support for clients in the form of case notes, applications for services, and paper file requirements
  • "No surprises" communication
  • Trauma-informed practices and communication with coworkers and clients
  • Active engagement at work to enhance program delivery and provide quality services
  • Initiative taking in continuing education and HIV-specific training
  • Able to work independently and as part of a team

Qualifications:

  • High school diploma or equivalent required, 2--5 years' experience working in social services preferred 
  • Must have driver's license
  • Bilingual applicants preferred
  • Candidates with lived experience with substance use preferred
  • Commitment to anti-stigmatizing communication with clients and client candidates
