Essential Functions of the position are detailed below, and include any physical requirements below: 1.Provides case management services to pregnant and parenting teens and their children in high risk areas with the goal of promoting adolescent and infant development, healthy family functioning and appropriate parenting. Maintains a caseload of 20-25 participants. Conducts intake and assessment, home visit with clients and parenting group sessions. May work evenings and weekends.2.Completes participant care plan and updates every six months. Enters participant's information in the database system and updates information as necessary within the prescribed time frames. Provides accurate and timely recording of all home visits, infant/child activities, vision and hearing information, immunizations and all other required data according to the guidelines, policy and procedures of the agency and the funder.3.Implements the Parents as Teachers evidence model in service delivery. Initiates weekly to bi-monthly home based services for all program participants within one week of referral.4. Provides services to clients, their infants, and families. Maintains an educational and supportive relationship with clients throughout their pregnancy, during the delivery and post-partum time period.5.Engages clients in services by greeting parents and children appropriately promoting a positive connection and a family partnership. 6.Facilities referrals to appropriate services when needed and/or identified by either the parent educator or participant.7.Prepares weekly and monthly activity reports on their community outreach activities for the hard to reach teen parent and networks with the community to inform them of program services. As a mandated reporter, immediately notifies the supervisor/director and the Department of Children and Family Services (DCFS) of any suspicions and findings of abuse or neglect.8.Identifies new resources and makes suggestions regarding utilization of current resources to the advantage of participants. Collaborates with service agencies on behalf of the participant. 9.Maybe assigned to conduct tours of the program, responds to inquiries from other agencies, visits agencies that are potential referral sources, and attends community meetings to build awareness. Participates in continuous performance quality improvement of services. 10.Utilizes s curriculum designed in 4 modules that provide supportive education information to the pregnant and parenting teens through weekly group interactions, at the area high school in the Maywood community that include pre and post test group evaluations. PLEASE NOTE: Essential functions include all other duties and responsibilities as assigned: Lift up to 25 pounds.Climb up and down up to 4 flights of stairs at a time.
