SUMMARY

Under general supervision, this Nurse Complex Case Manager coordinates and manages all aspects of patient care through multiple care delivery systems, acting as the central point of contact for patients, coaching their condition self-management, and supporting their adherence to their care plan. Receives case management reports and primary care physician (PCP) referrals in order to identify and engage very high and high-risk patients with whom to work, to support their optimal health outcomes, high quality care and patient safety, and reduce dependence on facility-based care. Through initial contact and assessment, establishes active partnerships with patients and clinical partnerships with PCPs, specialists, internal medicine (IM) leadership and staff, and internal and external resources. Reports to the Manager of Case Management, with a functional reporting relationship to the site IM Nurse Leader. Positions are also available in our Wellesley and Braintree sites.

EDUCATION/LICENSES/CERTIFICATIONS

Graduate of a state-approved school of nursing, Baccalaureate degree preferred (or equivalent education, training or experience). Certified Case Management (CCM) or CCM eligible strongly preferred. Must possess a current unrestricted RN licensure in the Commonwealth of Massachusetts. American Heart Association Basic Life Support (BLS) required. Advanced Cardiac Life Support (ACLS) may be required based on specialty.

EXPERIENCE

Minimum of 3 to 5 years nursing experience in case management/continuing care in an inpatient or community setting strongly preferred; exceptions in consultation with Chief Nursing Officer. Experience with an electronic medical records system, EPIC experienced preferred.

SKILLS

Must exhibit strong critical thinking, problem solving, interpersonal, and clinical assessment skills. Highly motivated and self-directed. Ability to interpret clinical information, assess the implication of treatment and develop and implement a plan of care. Ability to engage patients in adhering to a plan of care, and changing behaviors that impact their health. Ability to impact patient health behaviors and treatment adherence from multiple media – in person, via video, by telephone, thru messages. Demonstrated ability to prioritize, multitask, and work in a rapidly changing environment with multiple demands. Ability to work effectively in a team. Strong Computer Skills
