Uniontown Healthcare & Rehabilitation Center Licensed Practical Nurse in Uniontown, Pennsylvania

Position Summary:

The purpose of the licensed practice nurse (LPN) is responsible for rendering nursing care in assigned area in terms of individualized client needs, according to dependent and independent nursing functions and conformance with recognized nursing techniques, procedures, and established standards based on the scope of practical nursing, under the direction/delegation of the registered nurse.  *Essential Functions: * Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions. 

• Utilize nursing knowledge and skills in the safe implementation of basic preventative therapeutic and rehabilitative nursing care of assigned resident I patient as evidenced by documentation and observation of positive resident / patient care outcomes.

• Perform delegated independent nursing functions using established procedures, policies, guidelines and standards as observed by the registered nurse.

• Administer medication accurately, observing patient response, as evidenced by documentation in the medical record and lack of negative outcomes

• Maintain and coordinate communication among resident I patient I family, staff and physician as evidenced within medical record, client and health care team feedback.

• Assist in assuring that effective communication exists with resident / patient / family as reflected in the plan of care. Give punctual change of shift report consisting of pertinent and relevant information for assigned residents / patients.

• Work collaboratively with other members of the health care team in coordination of the resident/ patient's care as evidenced by other department feedback.

• Use available written and AV materials specific to patient condition or disease. Utilize appropriate teaching methods such as one-on-one and group instruction, demonstration or discussion.

•    Assist residents / patients, family members or other clients with concern and empathy; respect their confidentiality and privacy and communicate with them in a courteous and respectful manner. •    Answer and refer telephone calls or other inquires to ensure accurate and timely communications are facilitated.

Supervisory Responsibilities: * This position may have supervisory responsibilities related to CNA's.  *Work Environment: * While performing the duties of this job, the employee is in skilled nursing facility hospital  atmosphere that has controlled heat and cold. The noise level is moderate to quiet. The  incumbent may be exposed to residents / patients with contagious illness and must follow  established protocols regarding infectious disease control.  *Physical Demands: ( The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job)  While performing the duties of this job, the employee is regularly required to talk and hear.  This position requires a variety of activity levels including frequent standing, walking, bending, koeeling, stooping, and crouching, occasional sitting, rarely crawling and climbing. The employee may be required to frequently lift items up to 50 lbs.  Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception and ability to adjust focus.

•    High school diploma or equivalent required. •    Graduate of an accredited school of nursing. •    Valid registration as an LPN in the state with NAPNES or OOPNES Certification in Pharmacology. •    Completio ofrelevant state Board of Nursing approved training •    Current AHA Healthcare Provider training (BCLS) •    One year of experience in skilled nursing setting or medical / surgical nursing.
