Responsibilities:

Work with consumers in assessing health goals and implementing a Plan of Care.

Complete HARP (Health and Recovery Plan) assessments in order to link members to specialized community-based services

Assist members in addressing barriers to healthcare by providing health literacy education and addressing social determinants of health

Assist high-risk members in managing chronic behavioral health, medical and substance abuse conditions.

Link consumers to health and community resources.

Work collaboratively with health providers and other community service providers to promote primary and preventative care.

Assist consumers in reducing or eliminating the need for hospital/ER usage.

Manage all aspects of consumers electronic health records.

Promote a culture of inclusion and belonging.

Requirements:

Bachelor's degree in any field with a minimum of three years' experience in human services or Master's degree in human services and minimum one-year experience working within the field. Or CASAC and two years of human services experience.

Proficient with Microsoft Office. Prior experience with Medicaid delivery system preferred but not required. Must have reliable transportation. Valid and current New York State Driver License. Must have the ability to work in a fast-paced environment.
