Unity House Case Manager CHAP in Troy, New York

Position is eligible for a $1250 sign on bonus

Are you a passionate and engaging individual who can advocate for a diverse population who may not be able to advocate for themselves?

Please consider applying as a Case Manager for the Community Housing Assistance Program (CHAP).

Responsibilities:

Engage in ongoing and regular interactions with consumers.

Provide supportive counseling to consumers.

Provide daily living skills training.

Provide crisis intervention services.

Conduct home visits and provide transportation as needed.

Promote culture of inclusion and belonging.

Requirements:

Bachelor's or Associate Degree in health, or human services, or equivalent field experience preferred. Experience with multiple populations preferred. Must have regular access to a reliable vehicle along with a valid NYS driver's license. Bi-Lingual candidates encouraged to apply!
