Unity House Case Manager 8th Street in Troy, New York

Unity House of Troy is currently seeking a Case Manager in our Community Residence program located at 8th Street.

Position may be eligible for sign on bonus!

This job might be perfect for you if you enjoy working with individuals on a personal level and help them with the day-to-day routines.

Responsibilities:

Foster a structured and supportive environment.

Establish rapport with consumers.

Assist with daily living skills such as cooking, budgeting and personal care.

Assist with medication management.

Ensure satisfactory functioning of the facility.

Provide supportive counseling as needed.

Provide encouragement and giving positive feedback.

Transport clients and run program errands as needed.

Promote a culture of inclusion and belonging.

Requirements:

High School diploma or GED required and at least 1 yr of experience working with adults living with a mental illness. Valid NYS Driver License. Reliable transportation. Excellent verbal and written communication skills. Computer skills. Ability to work independently and as a team.
