Position Description

Nurse Practitioner - Haematology/Oncology Aftercare

Are you passionate about providing advanced nursing care to children, teenagers, and their families? Are you looking for an opportunity to use your advanced nursing practice expertise within an innovative, multi-disciplinary model of care to assist patients and families who have survived childhood cancer? Are you interested in not just the medical consequences of surviving cancer, but the psychological and social consequences as well? If so, we have an opportunity for you.

Here's What You'll Get To Do

In 2001, the Pediatric Oncology Group of Ontario (POGO) launched the provincial pediatric oncology AfterCare Program in collaboration with pediatric and adult cancer centres across Ontario. The AfterCare Program ensures systematic follow-up of survivors of childhood cancer from childhood through to adulthood. AfterCare clinics promote health and health education and monitor survivors at regular intervals for late effects of their cancer treatment. An important focus of this role is the assistance of patients and families as they transition to adult services.

  • Provide leadership and promote excellence in family centred nursing care through the five main role components of expert practice, education, consultation, research, and leader/change agent.

  • Integrate nursing and medical skills and knowledge within a broad scope of advanced nursing practice, with focus on post cancer treatment late effect risks (bone health, neurocognitive changes, heart health, endocrinopathies) and common primary health care challenges.

  • Assume a leadership role in influencing clinical nursing practice and the provision of care, collaborating closely with clinic team to provide post-treatment summaries/education and facilitation of multi-disciplinary Transition Clinics

  • Provide comprehensive health care to children and teenagers who have survived childhood cancer via NP-led and collaborative care clinics.

  • Coordinate the interdisciplinary plan of care for select patient populations.

  • Integrate research into practice, lead Quality Improvement projects focusing on improving access to survivorship education and supporting transition of care from pediatric to adult centres/providers.

  • Create, utilize and/or disseminate new knowledge.

  • Identify and link to services and resources in community.

  • Collaborate with experts in Fertility Preservation care and engage in care pathway development.

  • Participate in Provincial and Canada-wide collaboration with experts in survivorship care.

Here's What You'll Need

Essential

  • Master of Nursing or equivalent with a clinical focus

  • Completion of a Post-Master of Nursing (NP Field) Diploma

  • Registration with the College of Nurses of Ontario in the RN (Extended Class), in good standing

  • 3 - 5 years of clinical nursing experience in paediatric oncology

  • Innovative critical thinking with excellent communication, public speaking, and problem-solving skills

  • Ability to work with diversity, navigate complex situations and advocate for children and families.

  • Ability to act as a change agent, problem solver, critical thinker, team, and consensus builder.

  • Proficiency with Microsoft Word, Excel, PowerPoint and Windows and experience with Outlook

  • Demonstrated commitment and actions in advancing equity, diversity, and inclusion objectives.

Assets

  • Clinical knowledge and skills to manage complex nursing practice issues.

  • Ability to act as a change agent, and foster teamwork and consensus.

  • Knowledge of theoretical frameworks and research application

  • Experience with data collection and gathering/presenting metrics.

  • Evidence of innovation and visionary leadership to advance nursing practice and patient safety.
