ProMedica Health System RN / Registered Nurse Supervisor - Care Navigation - Full Time - Days in Toledo, Ohio

Provides direct supervision of the Care Navigators on the campus of Toledo Hospital. Ensures alignment of policies and procedures with Care Navigation strategy and goals. Promotes safe transitions of care for patients though operationalizing the core competencies of transitional care management, including relationship development, health promotion, comprehensive risk assessment and mitigation, connectivity with resources, and hand off.

ACCOUNTABILITIES 1. Contributes to the design, development and implementation of policies, protocols, and procedures to ensure standardization in process and promote operationalization of core competencies of transitional care management. 2. Responsible for on-boarding of Care Navigators and assimilating this role into the acute care clinical workflow management. Provides administrative and clinical supervision for Care Navigation. 3. In collaboration with Care Navigation leadership, oversees outcomes for Care Navigation program to align best practice, in support of quality, satisfaction, length of stay/capacity management, and utilization. 4. Remains knowledgeable about current regulatory standards that impact the Care Navigation department and process/workflows and contributes to process and policy alignment. Develops and implements audit tools and processes to ensure compliance with documentation requirements. 5. Utilizes coaching, mentoring, and education processes to ensure success of employee related to job functions. Contributes to employee evaluation process for Care Navigation. 6. Promotes connectivity between acute and ambulatory Care Navigation to ensure safe transitions of care for patients. 7. Contributes to special projects, as assigned. 8. Contributes and actively participates in the self-appraisal of the departmental effectiveness by providing input into the execution of the department goals and plans for improvement. Contributes to development of specific education and orientation programs to assist in meeting the department goals. 9. Other duties as assigned. Education: BSN Skills: Must have knowledge of regulations and community resources. Must be able to function effectively in a fast-paced environment. Written and verbal skills are essential. Must be able to establish priorities and communicate effectively. History of positive rapport with patients, families, physicians, and interdisciplinary team. Applicable experience in patient education and knowledge of hospital and community resources. Relevant experience in working with diverse populations. Exceptional problem-solving, critical-thinking, organizational, interpersonal, and written/verbal communication skills. Ability to work in a self-directed environment with attention to detail and follow-through. Computer literate; Microsoft Office competency. Years of Experience: At least 2 years of previous case management, care navigation or discharge planning experience. License: Active Nursing licensure in the state of Ohio.

PREFERRED QUALIFICATIONS Education: Master’s in Nursing or other relevant healthcare field. Years of Experience: 2 years of experience.
