Role Overview:

The ACP Facilitator works closely with the ACP Coordinator, palliative care team, case management, social services, healthcare providers, and other referral sources to address the advance care planning needs of the identified patients. Acting as a patient advocate, the ACP ensures that patients and families have a clear understanding of their healthcare options, enabling them to make informed decisions. Detail-oriented and thorough, the facilitator ensures all necessary steps are completed so that patients receive their chosen care.

Required Minimum Education:

  • Bachelor’s or Master’s degree in Nursing, Social Work, or a related field.

Licensure/Certification Required:

  • Respecting Choices® Advanced Steps Certification

  • License to practice in Ohio

  • CPR Certification

  • Ohio driver’s license and valid insurance

Minimum Years and Type of Experience:

  • At least two years of experience in palliative care, hospice, or similar consultative services for the adult/geriatric population.

  • Completion of or willingness to complete ACP Power Series in Workday.

Required Knowledge, Skills, and Abilities:

  • Proficient in Microsoft Word, Excel, PowerPoint and Outlook.

  • Ability to use DocuSign, RightFax and Perfect Serve.

  • Skilled in data reporting and documentation using spreadsheets.

  • Excellent interpersonal skills with the ability to communicate effectively with the multidisciplinary team and patients in a manner that fosters the team concept and supports the core values of the organization.

  • Flexibility and adaptability to change and frequent interruptions, with the capability to meet deadlines.

  • Ability to manage and prioritize multiple activities while working in a fast-paced environment.

  • Willingness to travel to multiple locations.

  • The candidate will have basic operational knowledge of the services provided by home care, hospice, and skilled nursing home stays and anticipated payor source(s) to be utilized.

Essential Job Function:

  • Collects data through observation, conversation, and chart review.

  • Conduct assessments to ensure decision-making capacity.

  • Establish and document legal healthcare decision maker accordingly.

  • Completes Advance Directives as necessary, providing copies, and uploading documents into EPIC and other electronic medical records as necessary; submits corrections to EPIC when identified as necessary.

  • Schedules and completes goals of care discussion utilizing Respecting Choices® resources and documents using BSMH’s smart phrase.

  • Completes DNR and POLST when relevant and obtains signature from healthcare provider.

  • Communicate patient information to multidisciplinary teams, including ambulatory settings, to ensure prompt attention to patient and departmental needs while remaining HIPPA compliant.

  • Educate patients on the differences and similarities between Advance Directives and portable orders such as the DNR-CCA, DNR-CC and POLST.

  • Provide conflict resolution and manage complex family dynamics impacting plan of care.

  • Conduct in-services in coordination with leaders and maintains continued education with staff and providers as needed.

Additional Functions:

  • Clerical skills/duties including but not limited to courier services, answering phones/call lights and orders, obtains, stocks, places, tests, discontinues, and removes patient equipment and supplies.

  • Communicates and Documents pertinent patient and general information and services provided.

  • Identifies, troubleshoots, and works with others as applicable to resolve issues and concerns.

  • Participates in hand-off/report of pertinent information regarding patient and departmental needs upon arrival and when leaving.

  • Other duties as assigned.
