Seneca County General Health District Health Educator II - Linkage Navigator in Tiffin, Ohio
POSITION: Health Educator II -- Linkage Navigator - Full Time (37.5 hours/week)
POSITION SUMMARY: The Health Educator II linkage navigator position, under the general direction from the Health Commissioner, is responsible for planning, implementing, and evaluating health education/promotion programs as assigned, with the overarching goal of improving the community's health through overdose prevention. This position will be Community based with a secondary focus within Public Safety to reduce overdoses through trauma mitigation. This position is expected to develop program acceptance, support, and collaboration with other individuals, community agencies, and other partners in order to achieve program objectives
ESSENTIAL DUTIES: Under close supervision, • Plans public health programs and activities as assigned by: o Assessing community health and mental health education/promotion needs for overdose prevention; o Involving community partners, staff, and target audience/population in program planning; o Developing measurable community/program goals and objectives; o Developing a logical scope and sequence plan for health improvement processes; o Researching relevant evidence-based and promising practices and selecting appropriate strategies and interventions for implementation; these will include trauma education and mitigation o Assessing factors that affect program implementation; o Preparing grant applications, reports and collecting/tracking data. • Respond to referrals for first responders, veterans and other community members who have experienced a recent traumatic event • Provide psycho-education regarding trauma and the correlation with substance abuse and other chronic disease health risks • Provide subclinical techniques to mitigate trauma i.e. G-TEP and A-TIP • Provide referrals for trauma specific counseling and/or substance use disorder treatment • Implements and evaluates public health programs and activities as assigned by: o Implementing selected strategies in collaboration with community partners, staff, and target audience/population; o Developing program and promotional materials and ordering supplies as needed; o Providing health education to community partners, staff, and/or target audiences/populations; o Planning, coordinating, and participating in community events, including fairs; o Completing and submitting required grant reports; o Monitoring program performance against objectives, identifying opportunities for improvement; and using quality improvement methods to improve performance. • Obtain, manage, track, train and distribute Narcan and Fentanyl Test Strips throughout the community. This will include installations of vending machines. • Maintain existing relationships with Promedica and Project Dawn. • Submit reports to ODH on a monthly basis • Serves as a liaison for the Health District in the community by participating on community coalitions, committees, and task forces as assigned. • Plans public health programs and activities as assigned.
