UTHealth Houston Neurosciences is hiring for a Patient Navigator to join their team of professionals. The Patient Navigator will be responsible for referral management, patient registration and communication with patients, the care team and care management vendor through the care coordinator process. Other responsibilities include collection and collation of outside medical records, patient transport coordination, and appointment scheduling. EPIC experience is a plus!

Position Summary:

The Patient Navigator engages patients, creates trusting relationships, navigates patients throughout their journey within the practice and closely communicates with the patient's care team based on goals and identified patient needs. The Patient Navigator will aid patients in the coordination and completion of appointments and help decrease barriers to timely care and treatment.

Position Key Accountabilities:

  • Oversees administrative functions of the program within the department.

  • Communicates and maintains a relationship with patients to ensure that appointment needs are met including facilitating transportation and/or financial assistance.

  • Maintains the integrity of patient information and the database, using HIPAA guidelines.

  • Researches new opportunities to enhance the program within the department.

  • Presents information within the practice to educate patients and staff about program offerings and services.

  • sMaintain regular communication with the patient's providers through clinical messages in EPIC and via emails, phone calls and case review meetings.

  • Makes recommendations for internal or external case management services when other issues are identified.

  • Works with medical interpreters to reach patients of other languages.

  • Produces mid-year and end-of-year reports on activities by compiling data from databases and writing up case examples.

  • Performs other duties as assigned.

Certification/Skills:

  • LCSW- License Clinical Social Worker preferred.

  • Strong communication and interpersonal skills.

  • Strong organizational and time management skills with emphasis on accuracy and attention to detail.

  • Builds awareness among the healthcare team and let them know about patient navigator services.

Minimum Education:

Bachelor's Degree or equivalent combination of education and experience required.

Minimum Experience:

  • 2 years experience in related field required.

Physical Requirements:

Exerts up to 10 pounds of force occasionally and/or a negligible amount frequently to move objects.

Residency Requirement:

Employees must permanently reside and work in the State of Texas.
