State of Massachusetts Mental Health Clinician - Clinical Social Worker A/B in Taunton, Massachusetts

The Department of Mental Health (DMH) is seeking a dynamic human service professional, exceptional and experienced Licensed Clinical Social Worker, Mental Health Clinician, to provide the day-to-day clinical support to a 47-bed adult residential program serving the Southeast Area. This individual will use their clinical expertise and assessment skills to inform the work of a multidisciplinary team of clinicians, nurses, mental health workers, peers, and student interns in meeting the mental health and daily living needs of each individual served by this program. The Mental Health Clinician will reflect the standard of practice and care of a program that is person centered and encourages each individual’s growth toward independence and recovery. The successful candidate will be someone who can work effectively with Program Leadership, Area leadership, Site Directors, and Residential Staff by working collaboratively with them in transitioning residents through the program. This person will also be required to demonstrate sound clinical judgment, have experience working with individuals who are mentally ill and dually diagnosed, have successfully implemented evidence-based practices. _Please Note_: This position is 40 hours per week. The selected candidate will work 8:30 am to 5:00 pm Monday through Friday. The days off will be Sunday/Saturday. _Duties and Responsibilities (these duties and responsibilities are not all inclusive):_ * Works as primary clinician for assigned persons served and is accountable for all service components provided by the team. * Responsible for the coordination and completion of the Comprehensive Assessment within the 45 day timeframe that is outlined by regulation by meeting with the client, family and any other individuals that can provide information as well as by gathering and incorporating outside documentation. * Responsible for the development of a Community Service plan for each person assigned that is completed within the 45 day time limit. The Community Service Plan development will include the person served, service providers and any other supports the person served would like to include. * Conducts ongoing assessments; actively participates in discharge planning; assumes primary responsibility for developing, writing, implementing, evaluating, & revising overall treatment/recovery goals in conjunction with DMH & Rehab Option guidelines. * Provide competency-based, evidence-based, and best practice interventions including the use of identified screening tools. * Responsible to ensure timely documentation of person served progress and to ensure compliance with existing DMH regulations and program procedures. * Provides culturally & linguistically competent services that are also sensitive and responsive to religious practices, sexual orientation, gender differences, physical disabilities, as well as services that are age and developmentally appropriate, including young adults and elders. * Serves on and provides clinical leadership to the GLE staff by attending regular team meetings, by coming prepared to present psychosocial history, mental status, diagnosis, treatment provided, and treatment planned for assigned cases to ensure that persons served receive optimal care. * Provides community based clinical interventions to persons served and side-by-side consultation/direction to GLE staff, in order to ensure successful provision of CSP interventions, to coordinate hospital discharges, to facilitate family meetings, and to access community services and support through care transitions. * Will provide individual, family and group therapy when clinically appropriate. _Required for this function: _ * Possession of a current and valid class D Motor Vehicle Operator’s license required for this function. _Preferred Qualifications:_ * Given the population served, bilingual or multilingual fluency. * * _About the On Grounds Program:_ On Grounds Program is part of the Taunton/Attleboro Site that transitions clients from an inpatient setting to a more independent community living setting.
