Position Purpose: Performs duties to related activities and campaigns to close open care gaps for high-risk members and improve healthcare outcomes.

• • Acts as SME for Outreach Coordinators and assists with the day-to-day training and mentoring. Provides back up support for Supervisor and performs UAT as needed. Performs duties to related activities and campaigns to close open care gaps for high-risk members and improve healthcare outcomes.
• • Conducts specialized and focused outreach and assessments for older members to close Care gaps Older Adults (COA) gaps
• • Provides support with other high-risk and aging members and/or providers to improve healthcare outcomes and close care gaps such as increased preventive care compliance
• • Conducts seasonal call campaigns, in coordination with local market efforts
• • Enters data assessments and authorizations into the system and maintain database as directed. Maintain integrity of Personal Health Information

This is a remote position available for applicants nationwide. Candidates will need to work within the Eastern Standard Time (EST) or Central Standard Time (CST) time zone.

Education/Experience: High school diploma or equivalent. Bachelor's degree preferred. 3+ years of managed care, medical office or facility experience. Knowledge of medical terminology, health care terms and conditions. Knowledge of medical terminology.

Pay Range: $22.36 - $38.07 per hour
