Job Description:

Responsibilities

The Public Health Representative IV will work to improve the health of Central New York communities in need by providing technical assistance, resources, and guidance on community/family health services to local health departments, community-based organizations, and other community partners within the 14 counties of Central New York. The incumbent will facilitate enhanced collaboration and coordination between Community/Family Health programs, providers, and other key stakeholders to improve health outcomes for infants, children, adolescents, and adults. The incumbent will work to improve the health of communities in need by providing training, technical assistance, resources, and guidance, to local health departments, community-based organizations, and other community partners regarding health education and health promotion activities including the cancer prevention/screening/survivorship/peer education, diabetes activities, the implementation of the Prevention Agenda and implementation of the Chronic Disease Action Plan. The Public Health Representative IV may be required to participate in public health emergency response activities as needed as part of the emergency response.

Minimum Qualifications

A Bachelor's degree in a related field and three years of relevant experience in a health, human service or community-based organization, that includes providing direct patient/client services, performing regulatory activities, conducting epidemiological research, and/or direct clinical care as a healthcare provider, or closely related experience; OR an Associate's degree in a related field and five years of such experience; OR seven years of such experience. A Master's degree in a related field may substitute for one year of experience. At least one year of experience must have included program management and/or supervision of staff.

Travel, 25-50% of the time, will be required. A valid driver's license in good standing will be required.

Preferred Qualifications

Familiarity with family health and/or chronic disease activities including infant, child, adolescent, and adult public health services; and prevention and control activities related to cancer, diabetes, cardiovascular disease, and/or asthma. Experience in oversight and evaluation of public health and preventive health programs, as well as promoting public health messages. Experience managing multiple tasks, developing, and delivering educational materials, coordinating training activities, and contributing on policy development. Excellent organizational, interpersonal, and written and oral communication skills. Ability to work both independently and as a member of a team. Proficiency with computer-based software packages such as Microsoft Office, Outlook, etc.

Conditions of Employment

Grant funded position. Compliance with funding requirements such as time and effort reporting, grant deliverables, and contract deliverables, is required.

Valid and unrestricted authorization to work in the U.S. is required. Visa sponsorship is not available for this position.

This position requires that the incumbent will report to the official work location and live within commuting distance to the official work location. Telecommuting will be available.

Travel, 25-50% of the time, will be required. A valid driver's license in good standing will be required.

This position may require occasional work on weekends, after-hours, and holidays.

Exposure to potentially hazardous substances may be involved with this position. Therefore, the incumbent in this position may be required to wear personal protective clothing and equipment.
