    Position Description:

    The LPN Population Health Support Specialist works as an integral part of an interdisciplinary team to gather information in support of provider, medical record documentation, upcoming appointment, and external sources for the non-provider portions of preventive wellness services for the identified patient population. This role will demonstrate compassion, collaboration, communication, and appropriate judgment in patient care. Assist in maintaining compliance with quality assurance standards and engage and support patients to ensure they feel heard, respected, and involved in their care. This is a remote position; however, the Specialist must live in Central New York.

    Requirements:

  • Education/Certification/Licensure

  • Current Licensure as a Practical Nurse in the State of New York.

  • Current Basic Life Support certification.

  • Experience/Skills/Proficiencies:

  • Knowledge of medical terminology, anatomy, physiology and pathophysiology.

  • Familiarity with health care systems, regulations, policies, and functions.

  • Understanding of documentation standards.

  • Knowledge of equipment, supplies, and materials needed for medical treatment.

  • Works well in a team setting and demonstrates strong cooperation skills.

  • Skill in reading and following written and oral medical orders.

  • Skill in initiating appropriate emergency procedures.

  • Skill in handling a number of tasks simultaneously.

  • Experience in a physician office, preferred.

    Pay Range: $18.50-$30.00 DOE
