Atlantic Health System Patient Navigator, AMG Epilepsy in Summit, New Jersey

Responsible for identifying critical resources for clients, helping them navigate through health care services and systems, and promoting client health.

Principal Accountabilities:

confirm insurance authorization, intake for referrals

  • notify doctor of any denials/appeals process

  • contact patient and schedule

  • patient admissions and attending EMU call should be matched per their established clinic provider, if possible (APP patients and outside referrals can go to any epileptologist)

  • ensure availability of doctors, rooms, techs for scheduled EMU daysneeds to be balanced with other outpatient EEG study needs

  • Notify EMU (nurse manager, nurse educator, charge nurse, etc) of any special accommodations

  • any outside referrals or patients with poor history should be scheduled for pre-EMU clinic intake visit (APP clinic, 1 hour visit), if possible

  • follow-up with patient to confirm (2 weeks, 2 days prior to admit date)

  • collect any additional notes/history/study results missing from referral info - (mainly from outside referrals)

  • send out calendar notifications to neurodiagnostics team

  • for phase II EMU admissions, must confirm with Neurosurgery team & OR coordinator - send reminders before admission day

  • ensure completion of EMU reports with the epileptologist

  • send completed EMU reports to the referring provider (if outside referral)

  • check which patients need post-EMU clinic follow-up and additional studie

  • send email to Clinic Navigator - for our psychogenic/non-epileptic patients

  • maintain log of EMU patients who are surgical candidates and their study results

Clinic Responsibilities:

  • Mon – Fri, 9 half day clinics per week + 1 half-day administrative time

  • include remote clinic site?

  • will see both new and follow up epilepsy patients, as triaged to the APP clinic

  • all patients (esp. during initial training period) to be supervised by epilepsy attending

  • sees VNS patients for post-op titration

  • sees new EMU referrals that have established outside neurologist, just need pre-EMU admit history intake

Monday EMU admissions (epilepsy service is admitting primary for phase I patients)

  • round on EMU patients daily in the mornings

  • report back to EMU attending on events. patient condition/concerns, questions etc.

  • check labs, adjust orders as needed

  • write daily progress notes

  • close communication with EMU nursing staff & EEG techs

  • first call for any EMU patient issues during the day, until 5 pm.

  • call EMU attending for any seizures for action plan

  • patient discharges (typically Friday mornings)

  • discharge orders, including any prescriptions

  • discharge summary

  • communicate with EMU coordinator & Clinic Navigator about follow up plans

Hospital Consults

First call for consult requests

  • Collect initial pertinent history and chart review

  • Report back to epilepsy attending for direction

  • See patient and start consult note

  • Follow up with epilepsy attending for action plan

EMU Coordinator

  • Review EMU referrals

  • call/meet with any clinic patients referred, for informational session

  • ensure insurance approvals/prior authorizations

  • schedule patients and manage EMU calendar

  • coordinate with NEREG EMU schedule

  • coordinate with Neurosurgery team for phase II schedule and OR calendar

  • keep track of EMU attending & PA call

  • follow up with scheduled patients to ensure proper preparation and admission reminders

  • communicate with Neurosurgery coordinator/scheduler

  • any phase II plans

  • Wada schedule, including epilepsy attending & neuropsychologist call

  • schedule for VNS/RNS/DBS placements

  • notify epilepsy attending of OR dates (RNS, DBS)

  • communicate with Clinic Navigator for patient follow up to be scheduled in epilepsy clinic

  • add to RNS/VNS/DBS clinic patient pools

  • schedule any new VNS patient with APP titration clinic

  • coordinate any patient guardian or transportation issues for EMU

  • communicate with hospital bed management for scheduled admissions

  • communicate with EMU nursing team

  • any admission/discharge issues

  • any concerns/questions to forward to the epilepsy attending

  • coordinate EMU nurse & MA seizure response/safety training sessions

  • (communicate with Dr. Yum)

  • keep EMU patient log

  • coordinate Epilepsy Conferences

  • keep conference patient log

Required:

  1. High School Diploma or equivalent.

Preferred:

  1. BCPA Certification.

  2. Experience and knowledge of all aspects of the healthcare system.
