Description

The Pediatric Nurse Practitioner will provide direct care to patients in the Division of Pediatric Primary Care Medicine at Stony Brook Children's Hospital and at the satellite practice locations in collaboration with an Attending Physician. The Nurse Practitioner/Clinical Instructor will be responsible for the following:

  • Provide coverage with physician back-up one weekend a month.
  • Provide education of staff nurses.
  • Confer with patient(s) to obtain accurate information for records, explain procedures, allay fears and elicit cooperation and engagement in plan of care.
  • Ensure execution of provider established plan of care including but not limited to follow up visits, specialist consultations, diagnostic lab, imaging studies and/or procedures.
  • Contribute to patient care through patient and family education, population health management, encouraging patient self-management, distributing resource literature and community service recommendations and referrals as needed.
  • Provide other required support to the Hospital-Medical Home grant implementation.
  • Assist in the annual review Nurse Practitioner protocols.
Qualifications

Required Qualifications:

  • New York State licensure as a Nurse Practitioner.
  • Master's Degree (foreign equivalent or higher) in Nursing.
  • Completion of a course for a Nurse Practitioner and certification as a Pediatric Nurse Practitioner.
  • Minimum of one year of clinical experience working in Pediatrics.

Preferred Qualifications:

  • Five or more years of clinical experience working in Pediatrics.
