Position Status:Full-Time (40 Hours Per Week)

FLSA Status:Non-Exempt

Pay Rate/Benefits: The pay range for this role is generally $41,600-$44,000 annually, commensurate with relevant experience and qualifications and in alignment with internal equity. Community Health Action of Staten Island (CHASI)drives dramatic improvements in the health of New Yorkers by feeding people who are hungry, healing families broken apart by violence, and bridging the gaps between people and the compassionate health care they deserve. CHASI provides outreach, education, prevention, and direct support services for populations most affected by health disparities – people with low or no income, low-income people with chronic illnesses, people with criminal justice involvement, people who use drugs, domestic violence survivors, people of color, and the LGBTQ community.

POSITION SUMMARY:

The Benefits Navigator is responsible for providing health insurance enrollment and SNAP (food stamps) benefits to uninsured and low-income Staten Island residents. The Full-Time Benefits Navigator will work from our Port Richmond location but will also attend clients in various other sites as scheduled. The schedule for this role is Tuesday-Saturday.

DUTIES & RESPONSIBILITIES:

Client Assistance:

  • Provide guidance and support to clients in understanding the eligibility requirements and application process for SNAP benefits and health insurance programs.

  • Conduct interviews and assessments to determine clients' eligibility for assistance programs.

  • Assist clients in completing application forms accurately and efficiently.

  • Address inquiries and concerns from clients regarding their benefits, eligibility status, and application process.

Eligibility Determination:

  • Review documentation and verify information provided by clients to determine their eligibility for SNAP benefits and health insurance coverage.

  • Stay updated on changes in eligibility criteria, program regulations, and policies related to SNAP and health insurance programs.

  • Collaborate with other agencies and organizations to ensure seamless coordination of services and resources for clients.

  • Outreach and Education:

  • Conduct outreach activities to raise awareness about SNAP benefits and health insurance programs within the community.

  • Organize informational sessions and workshops to educate individuals and families about the availability of assistance programs and how to apply for them.

  • Develop educational materials and resources to facilitate understanding of program requirements and benefits.

Case Management:

  • Maintain accurate records of client interactions, applications, and eligibility determinations.

  • Monitor the status of applications and follow up with clients to provide updates and assistance as needed.

  • Collaborate with colleagues and supervisors to address complex cases and resolve issues encountered during the application process.

Compliance and Reporting:

  • Ensure compliance with program regulations, confidentiality requirements, and data security protocols.

  • Prepare and submit reports on program activities, caseloads, and outcomes as required by funding agencies and stakeholders.

  • Participate in training sessions and professional development opportunities to enhance knowledge and skills related to SNAP and health insurance programs.

Requirements

QUALIFICATIONS:

  • At least 2 years of experience with relevant health care navigation, benefits navigation, and/or case management

  • Bilingual (English/Spanish) Spanish required.

  • High School diploma or equivalent required; Bachelor’s or Associate’s Degree, preferred.

  • Strong socio-cultural identification with the target population.

  • Proficiency with computers and web-based applications, MS Word and Excel.

  • Must be able to work a Tuesday-Saturday schedule.
