M Health Fairview has an immediate opening for a Team Coordinator at the Highland Park Clinic located in Highland Park Clinic, MN. Join M Health Fairview, the largest healthcare system in the state! The M Health Fairview Highland Parkl Clinic is a growing and collaborative clinic. Our clinic hours are 7AM to 6PM Monday - Thursday and 7AM - 5PM on Fridays, so no evenings, weekends, or holidays. This provides an excellent work life balance!

This is a 1.0 FTE (80 hours per two week pay period) opening and is benefits-eligible.

Responsibilities Job Description

As an integral member of the primary care team, the team coordinator contributes to overall quality of care and patient experience through performing high-level functions to support the needs of the healthcare team and patients/families. We have TC's partner with all members of the team to ensure patients needs are optimally addressed. They will deliver direct support to patients on the coordination of medical information requests and updates, as well as referrals and appointment scheduling. The TC will use a variety of communication tools and systems in their work to support patients and their care team. Additionally, we ask the TC's to maintain complete, timely and accurate documentation for each patient. They will also continually look for opportunities we can improve processes and workflow.

  • Performs telephone & MyChart message support & follow-up communication in a timely and professional fashion; uses communication systems for both routine & emergency transactions

  • Schedules, reschedules & maintains patient appointments & supports clinic leadership in schedule management

  • Partners collaboratively with care team to address patient needs

  • Find opportunities for improvement in workflow & participate in clinic and system meetings

Qualifications

Required Qualifications

  • Successful completion of formal medical training program such as MA, HUC, NA or Medical Secretary, or minimum of 1 year comparable experience in a healthcare setting

Preferred Qualifications

  • 2 or more years’ experience in a medical setting

Desired Skills

  • Knowledge of medical terminology and abbreviations preferred
