Position Overview:

Yes You Can Home Help Agency (YYC) is seeking a dedicated and trusted Community Health 

Worker with a strong connection to the community and a demonstrated understanding of the individuals we serve. This role is essential for promoting health within the community by providing a variety of services including outreach, health education, care coordination, and basic health screenings.

Key Responsibilities:

• Provide health education to individuals and groups within the community.
