Summary:
Exercises independent judgment in initiating and facilitating the flow of patient information and services for agency patients; communicates with clinical staff, clinical management, patients, and families to plan and implement scheduling and staffing changes as well as adding protocols to individual care plans. Responsibilities include obtaining intake admission information on new patients, scheduling clinical staff visits when indicated, addressing patient concerns/needs, obtaining physician admission orders for services, and collaborating with field staff to ensure client care needs are met in a timely manner. Responsibly directs and supervises, other members of the patient care team such as LPNs and HHAs. Provides direct patient care as needed. Performance reflects the mission and values of Home Care & Hospice.
Responsibilities:
Acts as the primary contact for team coordination
Processes physician orders
Obtains referrals and assigns clinical team member to patient, utilizing clinical knowledge to support clinical team needs
Applies clinical knowledge to triage patient symptoms and provide education telephonically and if necessary, in person
Responds to patient and family needs and takes appropriate action
Assists in researching patient concerns, following through with resolution
Collaborates with clinical staff and management to correct identified problems and trends
Collaborates with clinical leader to prioritize visits that are un-assigned due to staff absences and fluctuations in visit volume
Coordinates medication arrangements with Hospice pharmacy provider and hospice staff
Engages system Resource Center regarding Hospice DME, and patient/family needs
Utilizes critical thinking, clinical skills, and problem-solving skills to promote continuous quality improvement in supporting the patient care experience, scheduling and patient care coordination
Generates and distributes team reports as directed
May supervise and direct patient care provided by other team members
Participates in coverage of department seven days a week, including holidays
Other duties as assigned
Other Information:
BLS for Healthcare Providers certification is required to be obtained prior to independent visits.
Valid driver's license with reliable transportation and agency required auto liability insurance. Must be able to drive within assigned region as directed
