About the Role:

The LPN at JFHC plays a vital role in delivering comprehensive patient care, focusing on preventive medicine, health maintenance, and chronic disease population management. You will work closely with medical staff to ensure efficient patient flow, coordinate care across the continuum, and provide patient education under the supervision of a medical provider. This position also involves participating in quality improvement initiatives and may include oversight of students and allied health professionals.

Essential Functions:

  • Efficiently manage patient care progression, including vital signs and rooming patients.
  • Perform nursing procedures, therapies, tests, and services as ordered by medical staff and within your scope of training.
  • Champion a safety-conscious work environment.
  • Assess, plan, implement, and evaluate individualized nursing care appropriate to patient age.
  • Communicate effectively with patients, respecting their age, sex, gender, race, language, or national origin.
  • Prioritize and ensure timely care for all patient encounters (face-to-face, telephone, written requests).
  • Collaborate with medical staff and Medical Assistant to maintain provider schedules, efficient patient flow, and complete patient care.
  • Ensure smooth coordination and continuity of care for JFHC patients across levels of care, services, and locations.
  • Assist with chronic care management (depression, diabetes, CHF, chronic pain).
  • Provide general nursing care: medication administration, waived testing, EKG, and treatments.
  • Prepare patients for examinations, screen for information, and instruct on sample/specimen collection.
  • Educate patients/families on lab results, health maintenance, wellness, nutrition, medications, procedures, and treatments (under provider supervision).
  • Assist providers with procedures.
  • Observe, record, and report patient conditions and reactions to drugs/treatments.
  • Obtain and document patient history.
  • Maintain and review patient records and pertinent information.
  • Perform data entry and documentation of clinical information.
  • Respond to patient treatment phone calls and provide medication/treatment instructions.
  • Practice nursing within scope of license, standards of care, and clinic protocols.
  • Adhere to JST and JFHC policies and procedures (safety, work practices, HIPAA).
  • Perform additional duties as assigned.

Requirements:

  • Graduation from an accredited school of nursing.
  • Valid Washington State LPN license.
  • First Aid/CPR certification.
  • Knowledge of professional nursing theory, regulations, and practices.
  • Knowledge of safety hazards and precautions (infection control, environmental safety).
  • Skill in record keeping and written communication.
  • Clinical competence in patient care.
  • Ability to maintain quality nursing care standards.
  • Exceptional interpersonal and communication skills (verbal and written).
  • Basic computer knowledge.
  • Valid Driver's License.
  • Successful completion of criminal background investigation.

Preferred Qualifications:

  • Clinic experience.
  • Experience and knowledge of EPIC.
  • American Indian/Alaska Native preferences apply.
