Responsible for patient safety with primary responsibilities related to support of the Swedish patient safety team as well as support of the Culture of Safety Initiative.

The patient safety role includes being a member of Swedish organizational response to quality variances and having or developing skills to evaluate variance reports, perform root cause analysis and Healthcare Failure Mode Effect Analysis and investigations for serious safety events in support of the patient safety program. Participate and lead quality and safety meetings throughout the organization. Review and analyze safety trends and help guide strategic planning and prioritization for quality and safety related initiatives.

  • Identifies high-risk areas through Root Cause Analysis (RCA), case investigations, e-QVR referrals, phone referrals, incidents/accidents, and other relevant data sources.

  • Conducts Root Cause Analysis (RCA) to identify “root causes,” risk reduction strategies, and improvement strategies.

  • Conducts interviews with physicians, independent practitioners, managers, clinical nurse specialists, and staff in order to identify case specific details and to develop a complete understanding of events.

  • Develops support systems for staff and physicians involved in unanticipated events.

  • Reviews records, researches cases, and creates formal documentation of adverse events and sentinel events.

  • Convenes and facilitates the RCA Analysis Teams and RCA Review Committee to validate the root cause and action plan and to ensure that accountable individuals are assigned process improvement responsibilities.

  • Communicates with senior leaders regarding reportable adverse events and provides information as needed.

  • Manages event reporting in compliance with regulatory requirements and organizational policies and procedures, including reporting adverse events to the Department of Health.

  • Promotes integration of programs with Risk Management, Accreditation and Safety, Medical Staff Performance Improvement, and Patient Relations departments.

  • Provides leadership through participation in organizational planning, policy making, and consultation with a patient safety, risk reduction, and loss prevention focus.

  • Represents the department in system wide committees and makes referrals to mitigate risks such as write-offs, payment for follow-up services and care.

  • Collaborates and coordinates service recovery following events to minimize risk of claims.

  • Develops targeted educational programs in conjunction with RCA action plans.

  • Educates physicians and managers related to the RCA process and promotes compliance with organizational policies and procedures.

  • Develops abstracts and case studies from actual events to use for teaching programs for staff and medical staff, protocol development, and process improvements.

  • Continues to expand professional expertise by remaining current on regulations, technology and effective management practices.

  • Creates and participates in curriculum design for New Employee Orientation and Annual Education.

Required qualifications:

  • Bachelor's Degree in Nursing Baccalaureate degree (BSN) from an accredited school of nursing or must actively pursue and attain a BSN within six years of hire.

  • Upon hire: Washington Registered Nurse License

  • 2 years of Clinical experience.

  • Project management experience.

  • Experience in change management with internal & external customers including physicians, staff, managers, and directors.

Preferred qualifications:

  • Master’s degree in nursing, business, education, hospital administration Or equivalent educ/experience

  • Process improvement training.
