The Home Care Access Liaison coordinates and facilitates the non-clinical activities of in-home participant care by acting as a liaison between the PACE Organization and Contracted Agencies and IP/Caregivers to enable them to focus on quality care and participant satisfaction. Trains agency staff and IPs on the PACE program and ensures compliance with CMS regulations related to Personnel and Service Provision. The Home Care Access Liaison coordinates and maintains IP and outsourced agency caregiver schedules and medical records in collaboration with the Director of Clinical Services and HAWP Nurse; maintains participant confidentiality; maintains a hazard-free work environment. As appropriate, responds to participant requests and complaints; escalates requests/complaints to the appropriate staff as necessary. Works closely with participants, families, visitors, providers and staff to identify needs, answer questions, resolve problems, provide information and follow up. Responsible for coordinating and implementing Plans of Care and SDRs and Grievances according to the appropriate workflows. This position is a member of the Interdisciplinary Team, pursuant to 42 CFR §§ 460.102(b) and (d), as the Home Care Coordinator role.

Hourly pay range: $26.85 - $39.31

Education - High school diploma or equivalent. Experience - Experience working with the elderly, home care agencies, home health agencies or PACE preferred. Other Requirement(s) - Valid WA State Driver's License and proof of insurance. Must have active CPR and First Aid Certification, or be willing to obtain it within 1 month of hire.
