UW MEDICAL CENTER - NORTHWEST SOCIAL WORK has an outstanding opportunity for a CASE MANAGEMENT ASSISTANT . The Case Management Assistant will be responsible for working closely with the Social Worker, Nurse Care Coordinator, Physician, and the patient to arrange follow-up appointments post-discharge and assist with coordination of the discharge planning process for patients.

WORK SCHEDULE • Per Diem • Day Shift

RESPONSIBILITIES: • Arrange hospital discharge follow up appointments and referrals • Assist in arranging home services, such as home health, wound vac, or in-home caregivers, based on information obtained from clinical staff • Assist in arranging practical needs and emergency assistance, such as transportation, parking, or local lodging • Demonstrate problem solving skills in working with resources outside the medical center • Process and send clinical updates to payors • Research and update community resource information as needed • Participate in the training of Social Work Students and new employees • Document interventions in electronic medical record

Ideal candidates have experience working in a medical setting, demonstrate strong multi-tasking and organizational skills, oral and written communication, and computer proficiency. Must work well with a variety of persons and teams and possess excellent customer service skills. A Bachelor of Social Work or a Bachelor's Degree in the behavioral sciences and two years of experience as a Social Work Assistant or equivalent OR an Associate of Arts Degree in the behavioral sciences AND four years of work experience as a Social Work Assistant or equivalent is preferred.

REQUIREMENTS: High school graduation and a minimum of 3 years administrative/secretarial experience is required, preferably in health care setting
