Pacific Medical Centers Care Integration Coordinator - PacMed Seattle - Accountable Care in Seattle, Washington

Description

Supports the population health team by entering/coordinating incoming referrals and tracking outreach. Makes outgoing calls/sends Mychart messages to schedule appointments, provide resources, coordinate care, and provide class and visit preparation instructions to patients. Answers incoming phone calls from patients, providers, clinic staff and resources. Compiles data and prepares/submits reports for program, care and organizational performance metrics. Schedules conference rooms/venues for education classes. Provides routine financial counseling, researches routine referral and billing issues, and completes registration and data entry for programs. Responsible for set-up of all equipment, furniture, and other resources needed for classes. Takes and distributes departmental meeting minutes. Assists with all front and back office clerical duties as assigned by the supervisor.

Required Qualifications:

  • Upon request: Driving may be necessary as part of this role. Caregivers are required to comply with all state laws and requirements for driving. Caregivers will be expected to provide proof of driver license and auto insurance upon request. See policy for additional information.

  • 2 years Health care experience in ambulatory practice

  • 2 years Driving experience

Preferred Qualifications:

  • Associate's Degree

  • Prior experience using Excel spreadsheets

  • 1 year experience with coding: ICD-9 or ICD-10, CPT, HCPCS

  • Prior experience with an EMR

  • Prior experience in liaison role for patients, providers and staff
