Responsibilities

  • Case management to assist overcoming the barriers to maintaining housing and family stabilization using Family Development strengths based approach. Triage caseload to ensure adequate support to households in crisis and high needs households.

  • Complete comprehensive assessments of household’s strengths and needs using agency tools and best practices. Develop goals and milestones with household participation according to the Brief Solution Focused treatment model and Family Development.

  • Conduct regular and up to date evaluation of progress and modify goals on a periodic basis to provide support for successful retention of permanent housing. Do this independently and effectively communicate and work with a team – both in-house and through case conferencing as appropriate.

  • Ensure comprehensive access to all of SCAP’s services, working with agency colleagues and community partners to fully integrate, streamline and coordinate services.

  • Conduct advocacy on behalf of households in a solution-focused manner and represent SCAP in a positive, professional manner.

  • Conduct weekly home visits and interact with customers at off-site locations to observe and support task completion, crisis amelioration and stability in housing.

Qualifications

Requirements

  • Bachelor’s Degree in social work, human services or related field, Master’s Degree preferred.

  • Two (2) years’ experience providing related services to families and individuals; one year with a Master’s Degree.

  • Knowledge of community based services, public assistance and other community resources and demonstrated skills in accessing these services appropriately. Must have the ability to learn quickly the resources available and be able to appropriately refer in-house to ensure full access to SCAP’s services.

  • Must have demonstrated written and verbal communication skills and employ appropriate boundaries, representing SCAP in a professional manner.

  • Must demonstrate ability to perform job responsibilities independently.

  • Must have a valid New York State driver’s license, have regular access to a car and be willing to travel to off-site locations.

  • Demonstrated computer expertise and be able to stay up to date on software used in best practices.

  • Background check is required through the NYS Central Registry of Child Abuse.

  • Knowledge and understanding of issues related to individuals and families in poverty. Ability to build upon the strengths of those we serve with diverse racial, cultural, social, educational and economic backgrounds, including high risk populations, to provide high-level services and lasting solutions to fit each individual’s and family’s needs.

  • Applicants must be fully vaccinated against COVID-19 and show proof of vaccination [or receive a first vaccination by their first day]. Exemptions may be available on the basis of disability, medical contraindications to the vaccine, pregnancy or pregnancy related medical conditions and for religious reasons. Any requests for an exemption will be reviewed by the SCAP Human Resources Director and determined on a case by case basis.
