Eliot is seeking a Nurse Care Manager to provide medical care coordination as part of its multidisciplinary Behavioral Health Community Partner (BHCP) team. The Nurse CM will provide and ensure medication reconciliation and engage with medical providers of all enrollees. Assist enrollees in engaging with medical providers, recommendations and other types of health/ medical supports. Provide education and consultation to enrollees and CP team members regarding medical, health and medication related topics. Perform medical tasks as needed and directed interventions to enrollees related to their treatment plan. Provide services and supports for up to 250 enrollees. Additionally, candidate will possess strong time-management, organizational and communication skills allowing for some work related tasks to be completed remotely

Eliot’s Behavioral Health Division provides an integrated and comprehensive network of services to adults with psychiatric disabilities that are evidence based, provide rehabilitation and hope to those in services. Eliot's Behavioral Health Community Partner (BHCP) program is an innovative Care Management approach for MassHealth members with complex medical, behavioral health and social needs who are part of the new Accountable Care Organization (ACO) redesign within Massachusetts.

Responsibilities:

  • Provide education, consultation and care coordination to active enrollees on the CP team.

  • Provide interventions and supports to enrollees regarding medical health, medication and medical topics.

  • Provide 1:1 training in specific individualized need areas for assigned enrollees.

  • Develop and conduct groups and trainings for enrollees in general areas of health and wellness including nutrition, exercise, smoking cessation, diabetes, and prevention.

  • Assist enrollees with accessing community resources that will support and assist with addressing needs and health wellness efforts.

  • Provide oversight, guidance and assistance to enrollees with understanding their medical conditions.

  • Meet with enrollees and CP team members to discuss any medical diagnoses and conditions, causes, symptoms, complications, and treatment options.

    Schedule : M-F, 9am-5pm

    Location : Hybrid with options to work in Saugus, Danvers, Concord or Lynn offices. Travel required for face-to-face meetings. Mileage reimbursed.

    Salary : LPN $58,000 OR RN $70,000.
