The Brookdale HealthPlus LPN Care Coordinator works alongside our Brookdale HealthPlus RN Care Partner as a liaison and collaborator between residents, families, team members and healthcare providers with in designated communities. Responsibilities
The Care Coordinator identifies and coordinates the implementation of services necessary to improve residents’ quality of life and to achieve an optimum level of wellness at Brookdale’s HealthPlus communities. Works in collaboration with the RN Care Manager, primary and specialty care providers to recommend resident-specific care plans, coordinate medically necessary services, and the delivery of care. Serves as a resident advocate and assists in the identification of gaps in healthcare coordination. Identifies healthcare needs and coordinates community and external care resources as needed. Communicates with managed care coordinators, insurance providers, and external healthcare professionals regarding resident needs, status, etc. (e.g. primary care providers, home health agencies, etc.). Responsible for coordinating resources and equipment required as needed.
Collects resident data to determine health functional, and psycho social status of residents.
Coordinates services required to enhance optimum wellness and manage chronic conditions.
Gathers resident's specific data to assist the RN Care Manager and HWD with developing a personalized, holistic care plan.
Collaborates with hospice, palliative and home health services/therapy as indicated.
Participates in collaborative care reviews (CCR).
As directed by the Case Manager, deliver training to non-licensed care providers at the community to ensure the highest level of care for the residents.
Provides resources for residents and families to make informed decisions regarding choices in meeting healthcare needs. Effectively advocates for the benefits of residing and receiving care and services at the community.
In collaboration with Clinical Coordinators where applicable, provides oversight, coordination and family communication during resident hospitalization and rehab stays. Assists with return to community and ensures protocols are being met where appropriate.
Communicates information in a timely manner to the appropriate care provider regarding changes in residents’ health/well-being, personal needs, risk management issues, customer service issues, family, or outside health care provider concerns.
Coordinates the continuity of care for those residents receiving home health care, hospice services, and other third party healthcare- related services.
Completes documentation as required by Brookdale or state regulatory agencies.

Qualifications
Minimum Requirements
High School Diploma required.
Minimum of 1 year of related experience in a healthcare setting required.
Certifications, Licenses, and Other Special RequirementsMedical Assistant Certification preferred.
