Location: Sanford, Florida (Non-Profit Sector)

Role Overview:

• Provide intake, outreach, and case management for pregnant women and families with young children.

• Educate on family planning and related topics.

• Use strong telephone and computer skills.

• Basic knowledge of Medicaid and pregnancy issues.

Key Responsibilities:

• Enroll participants in a home visiting program.

• Help find prenatal and pediatric providers.

• Provide referrals and follow-ups to community programs.

• Explain Medicaid and WIC benefits.

• Educate on family planning, safe sleep, and prenatal/postpartum care.

  Requirements

• Bachelors Degree in Health/Social Care or related topic.

• Proficiency in Microsoft Excel
