Description

We are looking for a Case Manager for a company in Long Beach/ San Pedro. As a Case Manager you will play a vital role in assisting homeless individuals in accessing housing and support services.

Responsibilities:

• Conduct intake assessments and develop housing plans for homeless individuals.

• Provide ongoing case management services, including goal-setting and monitoring progress.

• Connect clients with housing resources, including shelters, transitional housing, and permanent supportive housing programs.

• Collaborate with local agencies, landlords, and community partners to secure housing options for clients.

• Assist clients in accessing healthcare, mental health services, substance abuse treatment, and other support services.

• Advocate on behalf of clients to ensure they receive appropriate services and support.

• Maintain accurate and up-to-date client records and documentation.

• Provide crisis intervention and support as needed.

• Participate in team meetings, trainings, and professional development activities.

  Requirements

• Previous experience working with homeless populations or in social services is desirable.

• Strong interpersonal and communication skills.

• Ability to work effectively with individuals from diverse backgrounds.

• Knowledge of community resources and housing programs in San Pedro and surrounding areas.

• Empathy, compassion, and a commitment to social justice.

• Bilingual skills (English/Spanish) may be advantageous.
