Description

Volunteers of America Southwest is looking for a dynamic person to help create change in our communities through our Comprehensive Opioid Recovery Center (CORC). The CORC program is a unique program designed to assist individuals in dealing with opioid addiction. The CORC program is a multifaceted program that provides services that match our client's level of need. The program is fluid and dynamic in providing services that include inpatient and outpatient treatment, case management, supportive services, benefit enrollment assistance and supportive housing services. Our Outpatient Case Manager will provide outreach, direct support services and assist in the coordination and implementation of participant services. Monday - Friday shifts.

Essential Duties & Responsibilities:

  • Provide community resources and referrals to meet the needs of our clients.

  • Screen program applicants for eligibility via telephone intake and/or on a walk-in basis and/or during outreach services towards program’s target populations (including street and healthcare outreach).

  • Ensure all applicants are screened for SAMSHA grants eligibility.

  • Assist program applicants and/or participants with collecting and submitting documents as needed.

  • Track screenings and participant contacts in an accurate and timely manner.

  • Administer assessments to determine eligibility.

  • Provide data entry and update of databases in a complete, accurate, and timely manner.

  • Maintain accurate and up to date participant filing.

  • Maintain positive communications and relationships with his/her supervisor, participants, employers, partnering agencies, involved members of families and other individuals in the community.

  • Complete in a timely manner all logs, reports and participant evaluations as required.

  • Provide direct service as needed and required.

  • Perform all job-related duties as assigned.

Qualifications

Education, Credentials and Experience:

  • Position requires a high school diploma or equivalent.

  • Must be a credentialed AOD counselor or a Registered Intern

  • Ability to work across multiple systems such as probation, criminal justice, and task force for the homeless.

  • Strong knowledge of and relationships with community resources.

  • Excellent written and verbal communication skills. Bilingual in Spanish a plus.

  • Ability to manage multiple tasks in a fast-paced environment.

  • Strong documentation skills.

  • Proven success as a team player.

  • Must comply with all agency and contract requirements related to trainings, public health, certifications, reporting, etc.

  • Must possess a CA driver's license, insurance and vehicle for local travel if needed.

  • Must be able to pass a background check, TB test and drug test.
