Service Navigator

Are you looking for an opportunity to make a difference in the lives of youth and families? Are you passionate about staying well-informed on resources and services available for youth with emotional or behavioral needs? The Office of Coordinated Care & Regional Supports is looking for an experienced individual to fill their role of Service Navigator. Job Responsibilities:

  • Serve as an expert on the service continuum and directs DHHS (Department of Health & Human Services) staff to available services that will support youth and families.
  • Maintain and develop relationships with providers throughout the state; become familiar with what services they provide
  • Help DHHS develop resources where gaps are identified
  • Represent the Office of Coordinated Care and Regional Supports (CC&RS) in meetings relating to youth placement and service needs
  • Help to maintain an accurate provider database; train DHHS staff on how to use the provider database
  • Monitor office contracts and manage invoices; ensure that providers are meeting required expectations; meet with providers to evaluate changes
  • Support the Intensive Care Coordination Program with monitoring Purchase Service Agreements (PSAs)
  • Support the System of Care grant by contacting families who are referred to Intensive Care Coordination,but are not eligible for services,and direct them to other services in all areas of the state
  • Use both data and experience to drive decisions about service needs

Ideal Candidate:

  • 3-5 years of experience in public and private mental health
  • Knowledge of both DHHS contracted and non-contracted services throughout the state,and can direct youth,families,and stakeholders in how to access these services
  • Knowledge and understanding of available services for individuals with mental health needs
  • Basic understanding of DHHS continuum of care
  • Experience monitoring contracts
  • Good time-management and organizational skills

Retirement - Utah Retirement Systems (URS):

  • Employees who have any service credit with an employer covered by the Utah Retirement Systems prior to 7/1/2011 are in the Tier I Public Employees System.
  • Employees hired on or after 7/1/2011 and who have never been in the URS retirement system have two options for retirement in the Tier II System.
    • Tier 2 Hybrid Retirement System. Employer contributes an amount equal to 10% of your salary between both plans (pension and 401(k))
    • Tier 2 Defined Contribution Plan. Employer contributes an amount equal to 10% of your salary to a 401(k) plan only.  
  • Employer contributions to your 401(k) are vested after four years.

Paid Time Off:

  • Annual leave and sick leave are accrued at 4 hours per pay period each (4 hours annual,4 hours sick).
  • Annual leave amounts increas e with years of service.
  • We have 12 paid holidays.
