Early Childhood Health Program Specialist

(Health Program Specialist I)

Would you like to contribute to ensuring families and children have access to the resources they need to reach developmental milestones? *The Division of Family Health is looking for a driven,detail-oriented individual to fill their role as an Early Childhood Health Program Specialist. *Job Responsibilities

  • Provide community outreach at events,clinics,and with collaborating partners to meet new families and collect contact information and potential referral needs
  • Counsel clients and prospective clients; screen referrals,provide community outreach
  • Communicate on the phone daily taking referrals and assigning to a Care Coordinator in a professional and timely manner
  • Provide clients with information concerning rights,options,benefits,services,goals and expectations
  • Refer client/patient to internal care coordination services to meet needs of clients
  • Decide upon the need for additional data,information,etc.,and authorize the means necessary to obtain the required information
  • Enter data into a computer system,retrieve,and/or correct previously entered data
  • Conducts or participates in day-to-day development and monitoring of contracts,subrecipient agreements,and grants,in support of the larger program functions
  • Engage in finding new partners
  • Provide additional administrative support to the Program Coordinator.
  • Ensures compliance with applicable federal and/or state laws,regulations,and/or agency rules,standards and guidelines,etc.
  • Other duties as assigned

Minimum Qualifications

  • Experience in customer service
  • Experience working in an office environment
  • Experience performing outreach services and taking referrals,preferably in relation to health services
  • Ability to use computer systems to perform data entry
  • Valid Driver License

Preferred Qualifications

  • Experience working in the Early Childhood field,where children are screened for developmental milestones
  • Ability to speak,write,and read fluently in Spanish
