Case Management Specialist Do you have case management experience? The Utah Department of Health and Human Services needs you as a* Case Management Specialist to provide case management services to eligible Ryan White Part B clients. You will work with the most vulnerable to be lost to care or at risk of being non-adherent to their medications and those residing in rural locations. You will be helping those living outside the Wasatch Front to provide access to services that have presented barriers in the past. You will provide help in an outreach setting such as in private-public meeting spaces,in client's homes,over the phone or using other technology. You will respond to technical assistance requests and will provide on-site technical assistance to Ryan White Part B internal and contracted case managers. Under the direction of the Case Management Coordinator,the Case Management Specialist will conduct contract-monitoring activities on a regular basis. These activities include,case management case note reviews and session shadowing.
