The M Health Fairview Care Coordination team has an opening for a RN Care Coordinator to support patients within M Health Fairview Bloomington-Oxboro, Edina and Eden Prairie Clinics. 

This is a 1.0 FTE (80 hours per two week pay period), benefits-eligible opening.

This position is a mobile position structured as a hybrid schedule with two days on site and three remote days per week.  On site clinic location(s) could be rotated and discussed upon hire, providing flexibility.  Core hours are Monday-Friday 8-4:30pm.

Responsibilities Job Description

Seeking an RN who utilizes a unique skill set to meet patients and families where they are at in their life journey to help them achieve their healthcare and personal goals. This position requires the individual to work in collaboration with the patient, family, and care team members and utilize superb communication skills across the health network to ensure a smooth and effective workflow. This position requires ongoing knowledge and teamwork with identified community stakeholders to help patients achieve their goals and become connected with the supports they need. The Registered Nurse must display a positive attitude, show dependability, and remain flexible in a fast-paced healthcare environment.

This position will include, but not limited to, the following tasks:

• Complete phone/in person complex assessments that include complex medical and psychosocial needs;

• Complete required documentation in established EMR in a timely and professional manner;

• Develop, in collaboration with the patient and family, individualized care plans that include personal and healthcare goals;

• Provide education regarding community resources and disease related education; assist the patient/families to become connected with these supports;

• Knowledgeable of medical and psychosocial resources and assessments available for patients across the lifespan;

• Ability to work in a fast-paced environment with a variety of different healthcare professionals;

• Consultation with healthcare professionals and community stakeholders regarding the RN Clinic Care Coordinator Role

• Advocacy for patients and families who need additional supports;

• Ability to think on your feet and complete crisis interventions as appropriate.

Qualifications

Required:

• Licensed RN in State of MN.

• Three years of specialty experience doing care coordination. Or licensed RN with five years of clinical experience and ability to learn new specialty area.

• Basic Life Support Certification

Preferred Qualifications:

• Licensed RN with four year Baccalaureate degree.

• Prefer two years of RN experience in the specialty area or previous care coordination experience.

• Specialized knowledge of the specialty and conditions of served population

• Ability to adapt to change and engage in ongoing process improvement
